Georgia Department of Health Form 4400 Certificate of Scoliosis Screening |
The Georgia Department of Health requires that current 6th grade and current 8th grade students be screened for scoliosis. The DPH Form 4400 must be on file, regardless if the parent chooses to screen the student or chooses to opt out.
If this form is on file for a 6th grade student, and that student continues enrollment for 8th grade at GCA, the student must have the second 8th grade screening on file. The 6th grade screening does not continue to the 8th grade.
