| Legal Guardianship |
Georgia Cyber Academy requires enrollment to be completed by the legal guardian of the student.
To establish guardianship, GCA accepts the following documents
- Birth Certificate
- Hospital Birth Record
- Court Order Guardianship
- Court Order Adoption
- Court Order Temporary Guardianship
- Court Order Parenting Plan Agreement
- DFCS Order of Placement
- Court Ordered Establishment of Paternity
GCA does not accept
- Marriage License
- Application for Guardianship
- DNA test results
- Life Insurance Beneficiary Documents
For proof of temporary guardianship, GCA will accept the following documents
Non-Parental Affidavit of Guardianship
- Non biological guardians can fill this document in it’s entirety, sign and notarize to place on file temporarily. This is not a permanent proof of guardianship. This document is only to be used if you are temporarily appointed as guardian (preferably legal guardian signs) or that you are in the process of filing for guardianship of your student.
Affidavit of Parental Guardianship
- Biological parents who are not listed on the birth certificate can provide the Parental Affidavit form as you are in the process of obtaining a legal document, amending a birth certificate or establishing paternity.
