Georgia Department of Health Form 3231
Unless specifically exempted by law, Georgia law (O.C.G.A. § 20-2-771) requires a certificate of immunization on file for each child in attendance in any school or child care facility in Georgia with penalties for failure to comply.
Immunization requirements in Georgia
Students attending 7th grade, and students who are new entrants to a Georgia school in 8th through 12th grade must have the following.
- One dose of Tdap vaccine
- One dose of meningococcal conjugate vaccine
Additionally, all students entering or transferring into 11th grade must have proof of the following.
- A meningococcal booster shot (MCV4).
If you are new to Georgia, all out of state immunizations must be transcribed on the Georgia DPH Form 3231 by a physician for the form to be accepted.
Exemptions to Immunizations
Medical Exemption to Immunizations

A medical exemption must be marked on the Georgia Immunization Certificate (Form 3231).
- A letter from a physician, Advanced Practice Registered Nurse (APRN) or physician assistant (PA) attached to the certificate will not be accepted as a medical exemption. It must be marked on the certificate.

A medical exemption requires an annual review. Your physician must provide this every year with the medical exemption marked and the new expiration date.

Religious Exemption to Immunizations
- The Affidavit of Religious Objection to Immunization form should be filed with GCA instead of the DPH Form 3231.
- The Affidavit does not expire.

**GCA Does not accept the DPH Refusal to Vaccinate Form. According to the Georgia Department of Education, there are only two exemptions - Medical or Religious exemptions.**
