If you received an email as shown below, this means a document remains missing or unsubmitted for your student's enrollment at GCA.
The body of the email will detail what documents are missing and you can follow the instructions in the email to submit them. Please note that the record must be a legible and complete document in order to be compliant at GCA.

DPH Form 3231 GA Cert of Immunization
DPH Form 3300 GA Cert of Vision, Hearing, Dental, and Nutrition Screening
DPH Form 4400 GA Cert of Scoliosis Screening
You may submit any missing record referencing your student's GCA ID number to records@georgiacyber.org. Please do not submit missing documentation to your FEL.
**Until we receive the missing record(s), your family (all students in your household) will not be eligible to have an approved re-registration application.**
